Retatrutide is a lipidated “unimolecular polypharmacology” peptide designed to activate GIP, GLP-1, and glucagon receptors, aiming to combine appetite/energy-intake reduction (GLP-1/GIP) with increased oxidative metabolism/energy expenditure signals (glucagon). Its pharmacokinetics support once-weekly dosing (half-life ≈ 6 days). In a randomized Phase 2 trial in adults with obesity, 48 weeks of once-weekly retatrutide produced dose-dependent weight loss, reaching ~24.2% mean reduction at the highest dose, with improvements in multiple cardiometabolic markers; the most common adverse events were gastrointestinal (e.g., nausea/diarrhea), consistent with incretin-based therapies.
